LIPOEDEMA

For a long time, lipoedema was a neglected disease by doctors and was often regarded as an inevitable ailment. Only a few years ago, a rethinking of lipoedema began in Germany. Since then, women affected by this disease have increasingly received medical support and help.

 

What exactly is lipedema? We are talking about a hormone-related fat distribution disorder in women, mainly in the arms and legs. It is characteristic that hands and feet remain spared, and there is only the typical collar-like thickening on the ankles and wrists. The diseased fat is chronically inflamed and, if left untreated, causes a change in the architecture of the subcutaneous fatty tissue. If secondary lymphedema develops, this means chronic pain and - in the final stage - grotesquely deformed legs for a woman suffering from lipedema; the so-called elephantiasis.

The clinical picture is divided into three stages:


Stage 1: Finely knotted skin surface
Stage 2: Coarse knotty skin surface with severe dent formation
Stage 3: Severely hardened and already deformed tissue with pronounced bulges of fat

 

It is increasingly recognized that lipoedema, which has a high genetic component, is to be regarded as a "widespread condition" affecting many women as early as the onset of puberty.  Often hormonal factors can be associated with a change in the nature of the disease (puberty, pregnancy, breastfeeding, menopause), which is due to the fact that the adipose tissue in the affected women appears to be more estrogen-sensitive. 

Lipoedema cannot be treated with medication, nor is it curable. That is why it is classified as a chronic disease. Manifest lipoedema can only be minimally influenced by dieting or intensive sport. Many patients are of normal weight. However, overweight should be consistently reduced, since obesity also has a negative effect on lipoedema.

In addition to physical stress and pain, the disease represents a significant stigmatization for those affected, since even their immediate personal environment often has little understanding for the diagnosis of lipoedema, and patients feel ashamed when eating, changing clothes in public or having intimate contact, and of course suffer considerably from this.

Conservative treatment methods are usually limited to complex decongestion therapies, compression garnments that are difficult to put on and uncomfortable, and lymphatic drainage. On the one hand, this complex conservative therapy, which only combats the symptoms of the disease, ignores the living environment of young, working patients, for example. Secondly, this is designed in such a way that it would have to be continued troughout life.

On the other hand, the only sensible and sustainable therapeutic approach is medical, axis-aligned liposuction using water jet technology (WAL). This is also scientifically based in the current S1 guideline on lipoedema. This method has recently been recognized by health insurance companies as an effective therapy and in individual cases they cover a proportion of the costs of an operation. Unfortunately, they are only willing to do this in the late stages of the disease, when the maximum effectiveness of the treatment is no longer given. Although its progression can be slowed down at this point, the pathological remodeling of the subcutaneous fatty tissue with permanent damage to the lymphatic system has already irreparably begun at this stage. Patients benefit most from treatment during stages 1 and 2, when the costs for liposuction treatment have to be borne by themselves, because health insurance companies still refuse to cover the costs of treatment at this point in the disease. 
 

Due to our extensive previous experience, we at JJPS are well acquainted with guideline-based clinical treatment of lipedema and have advised and surgically treated a large number of patients. We know that, especially in the early stages of the disease, often within just one or two sessions, a liposuction that is axis.approriate can remove most of the diseased fat and thus prevent stigmatizing and painful chronic changes in the affected extremities.

 

How do you find out if you might be affected from lipoedema?

 

  • Do you often have the feeling that your arms and/or legs are heavy as lead , even without much previous effort?

  • You have an extreme tendency to bruises, sometimes you don't even notice where and when the bruise occurred?

  • Your legs are very sensitive to pressure , your socks or pantyhose constrict, you often feel pain in your legs without a recognizable cause?

  • Do you have the feeling that your legs are steadily thicker despite exercising and eating a balanced diet?

  • Do you have the feeling that the fullness of your legs and/or arms doesn't match the rest of your trunk, so you're looking increasingly disproportionate?

Preparation

​

Precise planning of the areas to be suctioned is important. These are precisely marked in the standing position. Before medical liposuction, you must not take any blood-thinning medication for at least 14 days before the procedure and you should refrain from smoking. There are also some contraindications that we will discuss with you in detail during the preliminary talk.

Technique

​

Depending on the suction volume, the procedure itself is carried out on an outpatient or inpatient basis in approx. 2-3 hours. Eighter local anesthesia plus sedation or general anesthesia is used so that the procedure is painless. In addition to experience, the right technique is important in the case of lipoedema in order not to aggravate any accompanying lymphoedema: Axis-aligned liposuction using the WAL technique. The fat is gently liposuctioned off with the help of a water jet. We guide the suction cannula as parallel as possible to the course of the fine lymphatic vessels. The volume of the removed pathological fat depends on the individual circumstances, quantities of 5-7 liters are not uncommon.

 

At the end of the procedure, a custom-made compression garment or panty is put on to prevent painful swelling of the treated areas and to aid in rapid healing.

Costs & Financing

 

The costs for a session of axially correct, medical liposuction using the WAL technique vary depending on the effort involved and are made up of the surgical procedure, the material, the anesthesia and the final examinations. The costs start at 4.000 euros per session, depending on the time of the operation and combinations with other procedures.

Risks

​

The procedure is now a routine operation within plastic surgery. Nevertheless, there can be risks with regard to possible secondary bleeding, wound healing disorders or unsightly dimples. The latter may require correction later.

 

For smokers there is an increased risk of wound fluid formation (so-called seroma) and other problems, so we strongly recommend not smoking for at least four to six weeks before and after the operation.

Aftercare

​

The success of treatment depends to a large extent on behavior after the operation. It is therefore important that you refrain from sports and heavy loads for a period of six weeks after the procedure. During the treatment, cavities are created in the subcutaneous tissue that have to be compressed by external pressure: Therefore, a special compression garment must be worn day and night for six weeks to prevent the formation of wound water (so-called seroma). After 2-3 days, the first check is carried out in our practice. Further follow-up checks follow. Since we work with self-dissolving sutures, there is no need to pull threads. You can shower again immediately after the procedure. From the second week, manual lymphatic drainage can significantly accelerate the reduction in swelling. Sauna sessions or bathing should be avoided for two months. The scars are fine and with the right care, they fade quickly and are almost invisible.
