BREAST REDUCTION

More and more women are suffering from breast volume that is too large for their body proportions, sometimes already at a young age. According to observations, this applies mainly to women that live in industrialized countries. The exact reasons for this are unclear. In addition to a genetic component, hormonal influences and a changed diet appear to be responsible.

 

The fact is that a breast that is too large very often leads to degenerative changes in the cervical and thoracic spine, causes poor posture and is therefore a painful burden for the women affected. This applies to pain in the chest itself, as well as muscle tension and pain syndromes in the head, shoulders and neck. Sometimes there are also skin changes in the submammary fold and permanent retractions of the bra straps on the shoulders. These are complaints that are disruptive during sporting activity and massively restrict affected women in general in their everyday lives. The shock load on the holding apparatus becomes even clearer if you imagine a large breast in motion (e.g. when jogging). Here the weight of the breasts with their suspension on the upper body and the strain on the spine increased by the movement come into play even more.

 

In addition to the functional aspects mentioned, many women with disproportionately large breasts feel uncomfortable or misshaped and are therefore often dissatisfied with the aesthetics of the breast and the influence it has on their femininity. This can be remedied by various surgical techniques by reducing the size of the breast and adapting the shape of the breast to the patient's individual wishes.

 

In breast reduction (so-called mammary reduction surgery), skin and glandular tissue are reduced, in contrast to breast lift (so-called mastopexy), which is primarily concerned with correcting the skin layer and repositioning of glandular breast tissue.

 

As with all breast-modifying procedures, the declared aim in this case is to form breasts that match the stature and look natural while preserving the feeling in the nipples and the ability to breastfeed. The so-called pedicle of the nipple and areola is decisive for these factors. In the case of extremely large breasts, this pedicle – creating a tissue bridge under the skin in which the nourishing blood vessels and nerves to the nipple lie – is not always possible. In this case, the nipples are then transplanted "freely". In most cases, however, a nipple transposition is possible without any problems.

Procedure

 

During our consultation we will discuss in detail with you questions about the incision, pedicle and the desired volume and of course we will also inform you about the risks and details of the after-treatment.

 

About a week before the planned date of the surgery, you will be informed and any necessary preliminary examinations will be carried out. On the day of the operation, we draw the relevant surgical steps together again. The procedure itself takes place under general anesthesia, which is carried out by our experienced anesthetist who monitors you throughout the operation. In the preliminary talk before the operation, you will get to know him and have the opportunity to discuss all relevant questions with him. After the procedure, the anesthesiologist will continue to ensure that you are pain-free. The operation itself takes about two to three hours, during which you sleep deeply. In order to drain off wound exudate, fine drainages are placed next to the chest on both sides. In most cases, they can be removed the following day.

 

Compression garments (a special corsetry bra) will be put on after the procedure while you are still in the operating room, so that you can go home after a few hours of regeneration in the patient’s room – as long as you are well and feel fit. Of course you can contact the surgeon at any time by phone. Larger breast reductions are performed as an inpatient with an overnight stay in one of our partner clinics. You should permit complete physical rest for about a week, after that is it possible to do light office work or something similar again. You will be fully back to social presentability after about two weeks. However, the special compression bra should be worn around the clock for six weeks. You should also refrain from exercising for the same amount of time. Showering is already possible on the day after the operation with a special shower patch.

 

All follow-up examinations are included with JJPS. This includes the follow-up examination in the morning after the operation, further examinations after around 5 days and the suture removal after 14 days. A further follow-up examination is required six weeks after the procedure. The final outcome check is carried out six months later. Before the surgery, your gynecologist should perform an ultrasound or a mammography of your breast (depending on your age and risk profile) to be on the safe side. The pain of a breast reduction can be moderated with normal painkillers.

Technique

 

Nipple pedicle is a crucial variable in breast reduction surgery. Depending on the volume of the breast, the type of skin and the desired new breast size, either a so-called "I-cut" or a so-called "T-cut" is used. In both cases, the excess glandular, fatty and skin tissue is removed. The difference between the two techniques lies in the position and extent of the scar. The choice of technique is made depending on the degree of skin relaxation. At JJPS we master all relevant techniques and find a "tailor made" solution that suits your needs.

 

The variant of the "Inner Bra" has established itself as a particularly elegant and sustainable technique. To create this “Inner Bra,” a section of excess skin is formed into two tight straps that are hung from the pectoral muscle and sewn together. In this way they create an additional, firm stabilization of the newly formed mammary gland downwards, which can prevent the mammary gland tissue from sinking again. The "Inner Bra" is particularly suitable for women with weak connective tissue who want to continue to have a fairly large breast volume.

 

In order to achieve perfect results, a combination of different techniques is often useful. This applies in the case of a pronounced breast asymmetry, where the breast needs to be tightened to adjust it. We will discuss further details and all technical steps with you during our consultation hours.

Risks

​

In the hands of a board-certified specialist in plastic and aesthetic surgery with proven experience in breast surgery, breast reduction is a low-risk procedure. As a result, it generally leads to a very high level of patient satisfaction. General risks such as infections, bleeding, bruising and minor wound healing disorders are relatively rare and easy to remedy.

Aftercare

After the six weeks described above, you can take off the compression bra and start physical activities again. Scar care with special products is important, which we will educate you about in detail during the consultation and explain how to use them. With good care, the scars will hardly be visible in about 9-12 months.

 

Your usual breast cancer screening at the gynecologist (including mammography) is of course still possible and should definitely be done regularly.

Alternatives

​

In order to understand together whether the breast volume should be reduced or whether only a skin tightening is necessary, the goal of the operation must be discussed in detail in advance. Questions to be answered are how the breast can be brought back to its original shape, for example after pregnancy or severe weight loss. If the breast size is painful, there is no sustainable alternative to surgical breast reduction.

Useful information

With a breast lift, an asymmetry of the breasts that is perceived as disturbing can also be corrected in a simple and elegant way. Breast reduction can easily be combined with other procedures, such as a tummy tuck.

Another positive side effect is that due to the reduction in breast tissue after a breast reduction, the individual risk of developing breast cancer is also reduced.
