Many people want their face to present a slim, V-shaped appearance with defined contours, especially when viewed from the front. For men, the focus is on a defined jawline, while many women attach importance to contoured, high cheekbones. Unfortunately, nature rarely distributes those attributes that correspond to the current ideal appearance of a face in our cultural circle. Even many slim and athletically active women and men do not like to put up with excess cheek fat in the lower third of their face (keyword "chubby cheeks"), because they think it makes them look unintentionally weightier and bigger.

 

Responsible for this appearance is the Bichat's fat body. This fat deposit is located directly under the cheekbone, encapsulated between the chewing and cheek muscles in an isolated pocket, and can be of varying prominence. With a small and safe surgery, we can provide permanent remedy for this problem and make your face narrower. In this procedure - called bichectomy - the aforementioned pocket is opened and the Bicha fat body is very carefully removed. The procedure is performed through the oral cavity under outpatient anesthesia, so there are no external scars. The aim of the procedure is a significant narrowing of the cheek area with improved definition of the facial contour.

Preparation

 

Before each intervention, a detailed and comprehensible facial analysis with photo documentation is carried out. Before the procedure, you must not take any blood-thinning medication for at least 14 days and you should avoid nicotine. There are also a few contraindications that we will discuss honestly and in detail when we get to know each other personally.

Technique

 

The procedure itself can easily be carried out under a gentle local anaesthetic, if necessary supplemented by sedation, so that it is painless for you. If desired, general anesthesia is also possible. Before the procedure, the exact surgical plan is drawn in and the fat pad is then completely detached and removed via a small incision in the oral cavity. The oral mucosa is then sewn up with a self-dissolving suture. This procedure takes no longer than 30 minutes in total and leaves no visible scars. The final result is visible after just a few days. Any swelling or bruising that may occur can sometimes take two to three weeks to subside. The result of the operation is permanent because the removed fat does not grow back. The scars in the mucous membrane in the mouth are virtually no longer visible after a few weeks.

The costs for a bichectomy are made up of the surgical procedure, the material, the anesthetic and the final examinations. The costs start at 2,400 Euros depending on the effort involved and the combination with other processes.

Risks

 

For experienced specialists in plastic and aesthetic surgery, the bichectomy is a small, very safe procedure with a great effect on the facial contours. Slight pain can be treated well with painkillers, while swelling and bruising almost always go away on their own after a few days. As with any operation, infections and wound healing disorders can occur in rare cases. To prevent this, we will prescribe a special antiseptic mouthwash for the days after the surgery to disinfect your mouth.

Aftercare

 

Since the duration of the operation, as mentioned, is relatively short, you can usually return home after about 2-3 hours and the final check by the surgeon. On the day of the surgery itself, we recommend limited bed rest at home with your head elevated. It is important to cool moderately for one or two days to reduce the swelling. After a week, there will be a check-up appointment at our practice. Until then, you should refrain from sports or heavy physical exertion in order to keep your blood pressure low and to minimize the risk of bleeding. In addition, you should switch to a soft diet for two to three days and take care not to mechanically irritate the stitches when brushing your teeth. The antiseptic mouthwash solution should be used 3 times a day after meals, without mechanically rinsing too hard. Most of the time you are fully socially acceptable the next day. You should only refrain from intensive chewing for another week. The threads are self-dissolving, the surgical site in the oral cavity is usually healed after 3-4 days.
